Physical Examination

All Students Participating in Sports must have a card on file at their school. Physical Exam taken April 1 and thereafter is valid for the following 2 years & exams taken before April 1 is valid for the reminder of that school year. & the following school year..An updated Health History Update form must be completed for each season a student participates in a sport.
